Transition to adult life for Children and Young people with SEND

Transition is what we call the journey from being a child to being an adult.

Although change can be a period of uncertainty for young people and their parents or carers, it can also be exciting. Young people may be leaving school and considering their plans for the future. This includes for employment, training or further education.

The Transitions process aims for young people and their families remain involved in the process. This means they are enabled to have as much choice and control as possible. In some cases, we can refer a young person to independent advocacy. They can support them through their Care Act assessment. This way, the young person can have their views represented for them. This will help to reflect their dreams and aspirations as they see them.

From the age of 14 the Children's and Young People Disability Service (CYPDS) will support the young person and their families. This includes with their preparation towards adulthood. The team will continue to provide services, but work will focus on their current and future needs. They will also consider the most appropriate pathway for services post 18.
